Evaluation of the Context of the Implementation of the Development Program for Five New Regional General Hospitals (RSUD) in Bandung Regency
Abstract
The program to build five new regional public hospitals (RSUD) in Bandung Regency was initiated as a strategic effort to improve equitable access to health services, especially in remote areas, as targeted by the 2021–2026 Regional Medium-Term Development Plan (RPJMD) (Bandung Regency, 2021). This study evaluates the program's implementation process, focusing on two operational RSUDs: RSUD Cimaung and RSUD Kertasari. The approach used was descriptive qualitative with the Stufflebeam CIPP evaluation model, focusing on the process dimension, which includes technical/administrative planning, actor involvement, inter-agency coordination, transparency, field constraints, budget implementation, and realization of development outputs (Stufflebeam & Zhang, 2017). The results indicate that the program implementation process in both RSUDs was relatively effective: intensive cross-agency coordination accelerated construction, resulting in the completion of the two RSUDs in 3.5 years (faster than the 5-year plan) (Anggara, 2014). Technical and administrative planning was implemented in accordance with regulations (feasibility study, permits, DED) (Bandung Regency Health Office, 2021), with primary funding from the Regional Budget (APBD) allocated in stages, supplemented by support from the central government's Special Allocation Fund (DAK) (Bandung Regency Health Office, 2023). Stakeholder engagement was well-established, including collaboration between the Health Office and the Public Works and Housing Agency (PUPR) and outreach to the local community. Transparency was maintained through internal oversight and community participation (e.g., through local NGOs), thus increasing accountability (Freeman, 2006). However, challenges such as limited human resources and facilities were encountered during the initial operation, particularly at the more remote Kertasari Regional Hospital (Ministry of Health of the Republic of Indonesia, 2023). These challenges were addressed through the gradual recruitment of contract workers and equipment assistance from the provincial government. Overall, the evaluation process indicated that program implementation was on track and produced the expected output (the hospital was established and serving the community), but continued improvement was needed (Stufflebeam & Shinkfield, 2007). Recommendations are given to maintain cross-sector coordination, increase information transparency, accelerate the procurement of infrastructure, and strengthen the strategy for providing human resources, especially for regional public hospitals in remote areas, to ensure optimal service.
